Sphatik is used if Venus is favorable and outside the 6th, 8th, or 12th houses; otherwise a new Rudraksha (5, 8, or 9-faced) is used; those with skin allergies to either can use a Hakeek rosary colored to suit their ascendant planet.

When should this sadhana begin, and how is the rosary prepared beforehand?

Begin on any Bhairava Ashtami (Krishna Paksha Ashtami of any month) — the day of the week doesn't matter. A Rudraksha rosary is washed with raw cow's milk and Gangajal and consecrated with a round of Shiva's Panchakshari mantra; a Sphatik or Hakeek rosary instead needs a Prana Pratishtha beforehand.

What items and setup are needed for the ritual — Yantra, rosary, clothing, and seating?

A Tantric Bhairava Yantra prepared by a scholar (or performing at a Shiva temple), an authentic Rudraksha/Sphatik/black Hakik rosary in a red Gomukhi bag, red or yellow clothing, a Kusha grass or red/yellow wool seat, and facing any direction except South.
